What are the shipping charges?

The cost is $18 to have your machine shipped to you within the United States unless you get the new SD501U (under-the-counter unit). Then it costs $36 to have your machine shipped to you within the US. Quicker shipping options are available at your expense. 

When is an alternate payer form required?

An alternate payer form is required when the name entered on the Applicant information section does not match the name on the credit card being used to purchase the machine. The alternate payer form is authorizing Enagic to charge your personal credit card for the person/business listed on the Applicant information section. You will also need to provide a copy of the front and back of your driver's license.

What is required for a company to get a machine?

If you are putting the machine in a company name you will be required to provide the following forms and documents.

1. Product Order Form/Distributor Agreement
2. W-9 with EIN#
3. Return Policy
4. Copy of Articles of Formation
5. Copy of SS-4 (application form for the EIN#)
6. Alternate payer form unless paying with the company credit card.
