1. Single Payment require (Distributor) - this set of forms are required when ordering a machine, becoming a distributor, and paying with one single payment.

2. Single Payment require (User)- this set of forms are required when ordering a machine for personal use only, no distributorship, and are paying one single payment.

3. Using Enagic Payment Plan Financing Options require- this set of forms are required when ordering a machine, becoming a distributor, and using a payment plan. (Other Financing Available: User please contact us or Distributor)

4. Purchasing the SD501U (under-the-counter unit) - this set of forms are required when ordering an under-the-counter unit, becoming a distributor, paying with one single payment.

5. Other Forms

Fax Cover Sheet - used when faxing order in to the Corporate Office for processing.

Alternate Payer Form - used when the applicant name doesn't match the name on credit card.

6. Price Lists - prices on all models available.

7. Canada Forms - forms to be used for Canadian Sales.

8. Australia Forms - forms to be used for Australian Sales.
